Community Case Management


Community Living Association’s Adult Community Case Management program is a service that provides comprehensive support to adults with intellectual disabilities and autism. This service is based upon the belief that all individuals, through self-determination, can achieve a quality of life consistent with the community in which they live. Supports are flexible and designed in a manner that recognizes people’s changing needs throughout their lifetime. Services are provided to those people who are eligible for receiving intellectual disabilities services as determined by DHHS. The role of our case managers involves working with participants and others that are identified by the participant, such as family members, in developing an individualized support plan and assisting the person to implement that plan. Case managers will work closely with the client and their family to assure his or her ongoing satisfaction with the process and outcomes of the supports, services and available resources while assisting in the identification and implementation of the support strategies that reflect the client’s personal vision for a desired life.
