ASSISTED LIVING HOMES

► CLA has 1-four person home, 2-five person homes and 1-six person home. These homes are all wheelchair accessible Section 21 licensed homes.
► Residents residing in Assisted Living Homes have access to work and Community Supports opportunities.
► Transportation is provided by the homes for Community Supports, activities, appointments and individually areas identified as a service need while living in the home.
► All Assisted Living Homes are intended to provide a home-like environment and assist with care and support as needed. The homes are licensed through DIIBS, inspected by the Fire Marshall for safety and surveyed bi-annually. Each home is equipped with wired in fire/smoke detectors, fire extinguishers and emergency floor plans with required fire drills performed for residents and staff training in safe evacuation procedures.

► There are four Assisted Living Managers that manage the homes. The direct support staff is supervised by the four Assisted Living Managers. Residents who live in the homes are assisted by the direct support staff 24/7, depending on their individual needs. The Assisted Living Managers are supervised by the Director of Residential Services/ ALH.
► Home Provider qualifications are: High School Diploma and/or G.E.D., previous experience working with Adults with Intellectual and Developmental Disabilities (I/DD) is preferred, and a current Maine driver’s license with a clean driving record. Background checks are completed on all new hires.
► Once staff is hired, training is provided through Community Living Association’s Career Enhancement Center. The training consists of Orientation Day 1, 2, 3, CPR/First Aid, all CDS modules, CRMA, and PAC.

Forest Springs

Forest Springs is an 8 person Group Home that serves adults with developmental and other disabilities in a home-like environment. This home encourages people to work toward their fullest potential by qualified staff who help foster independence in the following areas:

• Language Skills: including verbal, manual, and augmentative communication skills 
• Activities of Daily Living: eating and personal hygiene 
• Physical Development: recreational, occupational, and physical therapy 
• Social and Emotional Development: awareness of self, others, the environment, and integration into the community 
• Community Skills: awareness, utilization of resources, and skills for participation including entertainment, shopping, church, banking, etc. 

The staff of Forest Springs includes an Administrator, Assistant Administrator, QIDP, Social Worker, Service Coordinator, Consulting Dietician, RN Consultants, Developmental Training Coordinators, Developmental Trainers Assistants and Resident Assistants.

Harmony

Harmony is a 12-person ICF Nursing Home serving adults with developmental and other disabilities in a home-like environment.  Harmony provides:

∙         Assistance in developing independence in social, self-help, motor,
communication, and self-advocating skills

∙         Furnishings to meet individual needs

∙         Personal services for good health, well-being, and safety

∙         Supervised activities and programs to promote independence

∙         Consultation from various support services

∙         Administration of medication therapy as needed

∙         Housekeeping, meals, and laundry services.

Harmony provides quality medical care for medically fragile people. Harmony has on staff a Director of Nursing, RNs, LPNs, and CNAs. Our qualified staff helps foster independence in language skills, activities of daily living, physical development, social and emotional development, community skills, and independent living skills. 

Harmony is a one-level home with four private and four double bedrooms, 3 offices, a medication room, a kitchen, a large dining room, 5 bathrooms (one with a traditional tub, one with walk-in shower), a whirlpool room, a staff room, laundry room, and a back patio.
