What is Community Case Management?
CLA's Community Case Management (CCM) program provides comprehensive support to adults with intellectual disabilities and autism. Our case managers serve as advocates, coordinators, and partners, working closely with each individual and their family to develop and implement a Person-Centered Plan that reflects their unique goals, strengths, and needs.
We serve individuals in Aroostook, Washington, Penobscot and Piscataquis counties.

Person-Centered Planning
At the heart of our CCM program is Person-Centered Planning. This approach puts the individual at the center of all planning and decision making, ensuring that their preferences, strengths, and aspirations drive the services they receive.
Our case managers facilitate the planning process, bringing together the individual, their family, and other support team members to create a comprehensive plan that addresses:
- Daily living needs and personal goals
- Health and wellness coordination
- Community integration and social connections
- Employment and educational opportunities
- Housing and residential support needs
- Advocacy and rights protection
Our case managers are dedicated professionals who build lasting relationships with the individuals and families they serve. They coordinate across multiple service providers and systems to ensure seamless, comprehensive support.
